TRI-COUNTY HUMAN SERVICES, INC.
Services for Children, Adolescents and Families

Tri-County Human Services Center is a private, non-profit agency which provides mental health and mental retardation services to residents of Northern Lackawanna (Olyphant to Vandling), Susquehanna and Wayne Counties. Our main office is located in Carbondale with satellite offices in Honesdale and Montrose. Lackawanna-Susquehanna-Wayne Counties MH/MR Office via the county commissioners contract with us to provide mandated mental health and mental retardation services.

The agency provide the following mental health services to children, adolescents and families between the ages of 0-18.

CRISIS INTERVENTION & EMERGENCY EVALUATION

.

 Outpatient Services
Services include initial assessment, psychological testing, psychiatric evaluation and individual, family and group therapies.

Appropriate reasons for referral may include but are not limited to: 

  • depression
  • suicide
  • anxiety
  • divorce
  • child abuse
  • family difficulties
  • school-related problems
  • other mental health problems
  • psychological and psychiatric evaluations/consultation
 School Outreach Services
Counseling and support services for youth with emotional problems in participating schools occur on a weekly basis.

 Student Assistance Program
Consultation, assessment/treatment, education/prevention and aftercare group services are provided to adolescents who are at-risk for emotional problems in conjunction with middle and junior/senior high schools.

 Family Based In-Home Therapy Program
Team delivered in-home family and parent therapy is provided to those which a child at risk for being placed outside of the home.

 Case Management
Service include Intensive Case Management, Resource Coordination, Psychosocial Rehabilitation Services of Wraparound and access to Mental Residential Treatment.

 Partial Hospitalization Program
Day Treatment programs combining intensive clinical and educational services are available for children and adolescents who are moderately to seriously emotionally disturbed. individual, group and family therapies are provided. Programs are located at 9 sites throughout the tri-county area with each site having two clinical staff, a teacher, a teacher assistant and a maximum of 12 students. Programs and age levels accepted vary by site.

 Summer Partial Hospitalization Program
This therapeutic day program provides socio-recreational activities to children ages 5-13 who have emotional problems. The Program is conducted 4 days a week for 6 weeks at 2 sites: Merli-Sarnoski Park and in the Wallenpaupack area.

 Parent & Child Resource Center
This is a parent education program for at risk parents and their children which is located in the town of Susquehanna and operates 3 days per week. Parents and their children, ages 0-5 attend this program 1-2 days per week for 4 hours each day. Residents of Susquehanna County are eligible. The program is subcontracted to our Agency by Susquehanna County Children & Youth Services who screen all referrals to the program.

 Family Preservation Program
The Family Preservation Program is subcontracted to our Agency by Susquehanna County Children & Youth Services. The program provides counseling and support to families in Susquehanna County who have a child at risk for out of home placement. Services are intensive and crisis oriented with a duration of up to 3 months. Families are screened prior to referral by Susquehanna County Children & Youth Services.

Acceptance into mental health programs may be dependent upon the meeting of clinical criteria, staff/space availability, license guidelines, family cooperation and available funding. Waiting lists may be in effect for some programs.

Tri-County Human Services Center is an approved service provider for First Priority and Geisinger HMO plans as well as other insurance carriers. All services are based on a sliding fee scale which is determined by family income.
